What usually goes wrong
Custom code with no owner
Extensions, triggers, and reports written years ago by people who have since left. Each one is an unknown in the next upgrade.
Integrations wired to the database
Other systems read EBS tables directly, or EBS pushes files on a schedule. When a table changes, something downstream breaks and nothing alerts.
Upgrades that keep slipping
The upgrade is deferred because nobody can size the risk. Meanwhile the platform drifts further from support and from the tools the rest of the company uses.
Clean core
The principle behind the work: standard Oracle functionality wherever it fits, custom code only where there is a business reason for it, and integrations outside the core.
A clean core is a series of decisions, made one customization at a time, about what the business still needs and where it should live. Many companies find that a good share of their custom code can be retired or replaced with standard features that did not exist when it was written. What remains is smaller, documented, and moved to where it can be changed safely.

1Audit
A full inventory of customizations, extensions, interfaces, and the business processes that depend on them. Each item gets an owner, a purpose, and a note on whether anyone still needs it.
2Rationalize
Each customization gets a decision: keep as is, replace with standard functionality, rebuild outside the core, or retire. The result is a smaller, documented core that can be upgraded.
3Re-integrate
Point-to-point connections move into an integration layer with defined interfaces, monitoring, and change control. Other systems connect to the layer, not to EBS tables.
